Access to Care

Ohio Pharmacists Association (OPA) and Buckeye Health Plan have joined forces to launch a program that leverages pharmacists as disease management experts for vulnerable patients suffering chronic diseases such as asthma or COPD. It will begin operations at two federally qualified health centers in Cleveland and Cincinnati with other community partners, targeting targeted interventions, predictive modeling, and regular consultations by pharmacists.

This program seeks to expand access for Buckeye members to telehealth visits with doctors at no cost to themselves, including convenient phone or internet-based visits for medical consultation. These telehealth sessions offer timely care that’s safe and efficient and help ease hospital overcrowding while allowing members to interact with their physician in ways that fit with their preferences.

Buckeye and Walgreens Pharmacy have joined forces as part of an initiative by Buckeye to partner with pharmacies to enhance member health and well-being. It builds upon a pilot program launched at select Walgreens locations to support Buckeye’s respiratory and chronic disease management programs; pharmacists in this initial phase provided asthmatic and COPD patients guidance in using their inhalers properly while coaching on adherence; they also worked alongside primary care providers to identify patients at risk of non-adherence and identify specific patient targets for support.

OPA’s partnership will ensure all members can access all available benefits, such as the Barrier Removal Fund. Under this program, participating providers have shared access to grant dollars for removing priority disability access barriers identified by Centers for Independent Living or other community partners.
